Advanced Primary Care Management is a program that helps us stay more connected with you and your care. Medicare has developed this program to assist Primary Care Physicians caring for their patients. Medicare covers most of the cost for this monthly service, but depending on your insurance, you might see a small coinsurance amount. This is a patient-focused approach, where our team takes extra steps to help manage your health needs. Some, but not all, examples of such services include things like:

1. Creating a personalized care plan for you- outlines goals and needs of your healthcare.
2. Coordinating your medical care with your specialists and ancillary services.
3. Helping to ensure you receive recommended preventitive services during annual wellness visits.
4. Assisting with medication management, refills, and Prior Authorizations.
5. Communicating directly with Dr. Elias, Dr. Oakley, or our staff, through our patient Portal or on call services 24/7.
6. Follow up care coordination following emergency room visits or discharge from the hospital.
7. Virtual Visits via Telehealth.
